ENDOCARDITE INFECCIOSA: DESAFIOS DIAGNÓSTICOS E TERAPÊUTICOS NA ERA DAS RESISTÊNCIAS BACTERIANAS.

A endocardite infecciosa (EI) é uma patologia grave caracterizada pela infecção do endocárdio, frequentemente associada a elevada morbimortalidade. O avanço das técnicas diagnósticas e o surgimento de cepas bacterianas resistentes têm modificado o perfil clínico e o manejo terapêutico da doença, tornando seu diagnóstico e tratamento desafiadores. Trata-se de uma revisão de literatura narrativa, realizada entre maio e setembro de 2025, nas bases de dados PubMed, SciELO e LILACS. Foram incluídos artigos publicados entre 2020 e 2025, em português e inglês, que abordassem aspectos diagnósticos, terapêuticos e microbiológicos da endocardite infecciosa. A análise dos estudos evidenciou que o diagnóstico precoce depende da integração entre critérios clínicos, laboratoriais e de imagem, com destaque para o ecocardiograma transesofágico e a hemocultura. As cepas resistentes, como Staphylococcus aureus MRSA e Enterococcus faecalis multirresistente, representam um importante desafio terapêutico, exigindo esquemas antimicrobianos individualizados e, em alguns casos, intervenção cirúrgica. A endocardite infecciosa continua sendo uma condição complexa, cujo sucesso terapêutico depende do diagnóstico precoce e de estratégias antimicrobianas adequadas. O monitoramento das resistências bacterianas é essencial para otimizar o tratamento e reduzir a mortalidade associada à doença.
